Our Vision
The Coalition will collectively foster a
person-centered and directed model of care in Minnesota that offers individuals who live and work in long-term care settings, supportive communities that uphold individual dignity and respect and enable choice and self-determination.
The Minnesota Culture Change Coalition is a broad range of long-term care stakeholders working to support, supplement, and foster culture change in nursing homes on a broader scale than could be achieved through any one of the group’s individual efforts.
