Minnesota Culture Change Coalition Members
The Minnesota Culture Change Coalition is open to anyone interested in person centered care. The Coalition was founded in 2004 and a number of organizations have been active in the Coalition representing the long-term care industry in Minnesota and includes; provider associations, provider facilities, coalitions and consortiums, consumer advocacy groups, government organizations, quality improvement organizations, diagnosis focused organizations, professional membership and workforce organizations, educational organizations.
