Minnesota Culture Change Coalition Members
The Minnesota Culture Change Coalition is made up of a variety of organizations representing the long-term care industry in Minnesota and includes; provider associations, provider facilities, coalitions and consortiums, consumer advocacy groups, government organizations, quality improvement organizations, diagnosis focused organizations, professional membership and workforce organizations, educational organizations.

To apply for membership, submit a letter of interest to the Minnesota Culture Change Coalition that includes the following:
- Name contact of person organization requesting membership
- Mission statement of organization
